Payment Schedule and Options

Payment schedule and options are: A $300.00 deposit and application fee, due July 1, 2008 and:

1. Single payment: $2,200.00 due no later than August 1, 2008 (Total $2,500.00) or
2. Two installments: One of $1,100.00 due August 1, 2008 and one of $1,200 due December 1, 2008 (Total $2,600.00) or
3. Monthly Payments: An initial payment of $600.00 due August 1, 2008 plus 9 monthly payments of $200.00 due on the first of each month beginning September 1, 2008 through May 1, 2009 (Total $2,700.00).

• I agree to the above selected Payment Schedule. Enclosed is my check in the amount required by the Payment Schedule.
• I agree to provide the balance required not later than the dates set forth in the Payment Schedule.
• I further understand that if any payment is more than 15 days overdue, I will not be allowed to continue in the program until my tuition fees are made current.
• By submitting this application, I acknowledge and agree to the Payment Schedule above and understand that if I am admitted to the program, my $300.00 deposit will become non-refundable on August 1, 2008 and will be applied to my tuition.
• I further understand that if I am admitted to the program, any payments made or received on or later than August 1, 2008 are non-refundable.

Personal Information

Agreement to Terms

• I understand that, upon satisfying all of the requirements of the Vajra Pani Yoga Teacher Training, I will receive a 200-Hour Yoga Vajra Pani Yoga Teacher Certification and that the Vajra Pani's curriculum follows the criteria established by Yoga Alliance for certification at the 200-hour level.
• I further understand that, should I fail to meet all of the requirements for certification, I may "make up" the missed elements of the program on my own, at my own expense. In this case, the "make up" work will require documentation provided to the Vajra Pani Yoga Teacher Training Program Director.
• I understand that, in the extremely unlikely circumstance that my participation be deemed destructive, inappropriate or unethical, in the sole discretion of the Vajra Pani Yoga Teacher Training Program Director, I may be required to leave the program before its completion date and all amounts paid shall be forfeited.
• I further understand that all materials, written, created by the Vajra Pani Yoga Training and provided to me in the course of this program, may not be copied, reproduced or distributed, in whole or in part, or by any means without written consent of the Vajra Pani Yoga Teacher Training Program Director.
